Our annual Staff Awards recognise the efforts our staff go to in providing the best possible care and support to our service users, their families, and carers. 

If a member of our staff or a team has made a real difference to your life or that of someone you know why not nominate them for an award?

How to nominate

Online
Please follow the instructions below to use our online voting system.

  • Click here to access the online system. The system will ask you to register before you select your entry.
  • To register click on CREATE AN ACCOUNT once you have completed the registration you need to sign in.
  • You then click CREATE AN ENTRY.
  • Click in the category box and select Annual Staff Awards.
  • Select the award you wish to nominate for
  • Complete the entry form and click CREATE AN ENTRY.

By post
PDFs of the nominations forms appear under each category. When you have filled in the appropriate form(s) please send to: Communications Team, Hertfordshire Partnership University NHS Foundation Trust. Colonnades, Beaconsfield Road, Hatfield, Hertfordshire AL10 8YE.

The closing date for nominations is 1 October 2017.

Categories

Integrated Care / Partnerships Award
This new award is open to teams, individuals, services, and projects. This award acknowledges excellence in the delivery of integrated care by a team, individual or service. 

Nominations can be made by service users, carers, their families, staff and partner organisations.

Council of Governors - ‘Make a Difference’
The winner of this award will be an individual or team that have made a significant contribution to a service user’s personal recovery journey, working in partnership with the service user and their carer/family (where appropriate), demonstrating the skills and qualities that inspire hope for a positive future, in alignment with the Trust’s values.

Nominations can be made by service users, carers and their families.

John Lewis ‘Valuing Our Customers’ – Team
This award is given in association with John Lewis and will be presented to a team that has provided a consistently excellent customer experience, which aligns to the Trust’s values, and/or have demonstrated a significant improvement in the customer experience they are providing.

Nominations can be made by service users, carers, their families and staff.

John Lewis ‘Valuing Our Customers’ – Individual
This award is given in association with John Lewis and will be presented to an individual that has provided a consistently excellent customer experience, which aligns to the Trust’s values, and/or have demonstrated a significant improvement in the customer experience they are providing.

Nominations can be made by service users, carers, their families and staff.

Innovation and Improvement - Team
This award is for a team or project group that has implemented a continuous service improvement or innovation, however big or small, that has enhanced the service user experience and improved services.

Nominations can be made by service users, carers, their families and staff.

Unsung Hero/Heroine
This popular award is given to an individual who consistently exhibits the Trust values in their everyday work, going the extra mile to help service users and colleagues.

Nominations can be made by service users, carers, their families and staff.

Clinician of the Year
This award is for a clinician, who through their personal leadership and contribution, has raised the profile of their profession, or supported fellow clinicians, or participated in research and development that has contributed to improvements in the profession.

Nominations can be made by service users, carers, their families and staff.

Volunteer of the Year
The award is for HPFT volunteers only, who through providing their free time to support HPFT services, have made a difference to the lives of service users and carers and helped to enhance the delivery of HPFT services.

Nominations can be made by service users, carers, their families and staff.

Positive in Practice
This award is for teams or individuals who have supported their colleagues, enabling them to work more effectively, for example providing an exceptional support service, taking the time to resolve a situation, even if it is outside of their job role, or just always being willing to help. 

Nominations can be made by staff.

Leading our Values
This award is for an individual who demonstrates leadership and has inspired and motivated staff and their colleagues, consistently role modelling the Trust’s values.

Nominations can be made by staff.

Chair’s Inspire
The winner of this award is selected from the previous year’s Individual Inspire Award winners - individuals who have consistently demonstrated the Trust values in their work.
