NHS England published an independent investigation into the circumstances surrounding the homicide of Ms M by Mr O, a 26 year old male HPFT service user at the time. The NHS England Independent Investigation was published with the report from a Multi Agency Partnership Review.

Dr Asif Zia, Executive Director of Quality & Medical Leadership at Hertfordshire Partnership University NHS Foundation Trust, said: “I would like to offer my sincere condolences to Ms M’s family, her friends and everyone affected by this tragedy. We are very sorry for their loss.

“We have undertaken a thorough internal investigation into the care and treatment of Mr O while he was in our services. We have fully participated with both the Multi Agency Panel investigation and the NHS Independent Investigation. We accept the recommendations from both investigations.

“Mercifully, cases such as these are exceptionally rare. However, we appreciate the terrible impact, including on families and friends and the wider community, when they do occur.”
