Sustainability and Transformation Plan

 

In December 2016, ‘A Healthier Future’, the Sustainability and Transformation Plan (STP) for Hertfordshire and West Essex, was published setting out a five-year plan for health services in the area.

‘A Healthier Future’ recognises the need to shape services around service users with a joined-up approach between physical and mental health services.

This approach is in line with the way we have been working for some time. Our 'Good to Great' strategy clearly puts Great Care and Great Outcomes for service users at the heart of our work – ensuring that the service user and carer experience is the best it can be. It also underlines the need to work with Great Networks and Partnerships to deliver this.

Our alliance with Hertfordshire Community Trust (HCT) is a vital part of this partnership working, by providing joined-up care that meets people’s mental and physical health needs. Our work on the Home First/Rapid Response Service, the Multispecialty Team in Watford and the development of a shared health facility in Hemel Hempstead are all examples of this joined-up approach.

‘A Healthier Future’ supports our direction as a Trust and the way in which the Trust works. Whilst there will be no immediate changes to the way HPFT works there will be rather further opportunities for developing even better services for the people HPFT serves.
