'With every digital project, we are building a more sustainable and accessible way  for our service users and carers to engage with the Trust.'
HPFT Digital Strategy.

Censeo

Censeo is a new and innovative online initial digital assessment available to some of our working age adult service users. They can tell HPFT in their own words why they are seeking help and support right at the start of their journey with us. The assessment is optional and reduces the need for service user to repeat their story. It is accessed via an online link sent to mobile phones allowing the service user to complete it in their own time, at a pace and location comfortable to them without the pressure of a clinical environment.

Completion of the form generates a report which is sent to HPFT care professionals for review to help identify the best form of care.

To date the project has been positively received:

“Everything is important is covered as best as it is possible to be” – Service User

“This is incredible, I know often service users are asked to repeat their story time after time, it will really help with the service user journey and experience” – HPFT Staff
