Excellence in Partnership Award

This award celebrates outstanding achievement in bringing service users, carers, our partners, the community or our staff together in such a way as to deliver a significant improvement in the provision of high-quality services.

Award nominations for individuals or teams will demonstrate shared decision making, co-production and high levels of engagement.

If supporting evidence is included (photos, charts, graphs, etc) please indicate where/how the evidence supports the nomination submission.

Nominations for this award can be made by service users, carers, their families, our partners, community leaders and our staff.
