Transforming Lives Award

This award celebrates individuals or teams who have supported our service users to live independent and fulfilling lives.

This may be through building connections in their communities, by acquiring new skills and resources to enable independent living, by creating new opportunities or sourcing new options for community-based care in order to provide the very best outcomes for our service users, carers and their families.

If supporting evidence is included (photos, charts, graphs, etc) please indicate where/how the evidence supports the nomination submission.

Nominations for this award can be made by service users, carers, their families, our partners, community leaders and our staff.
