Agomelatine Named patient request only. Prescribing to remain with HPFT – NOT to be transferred to primary care. Risk of dose related hepatotoxicity and liver failure see MHRA updated warning and monitoring guidance Oct 12
Asenapine NOT approved.
Cariprazine NOT approved.
Clozapine IM Named patient request only. See HPFT Guidelines for inpatient use of intramuscular clozapine (on intranet). 
Dosulepin NOT Recommended (NICE CG 90) – cardiotoxic and toxicity in overdose.
Escitalopram Named patient request only. Citalopram and escitalopram: QT interval prolongation - new maximum daily dose restrictions (including in elderly patients) contrainidcations and warnings: MHRA Dec 2011
Esketamine Nasal spray NOT recommended for treatment resistant depression in adults  (NICE TA 854)
Melatonin NOT approved for short term treatment of insomnia in adults aged over 55 or for short term treatment of jet lag. (Approved for neurodevelopmental disorders in patients under the age of 18 years and for insomnia in adult and paediatric patients with a learning disability for approved preparations - see sleep disorders
Olanzapine Embonate LAI Named patient request only – risk of post injection syndrome. See HPFT Guidance on the use of Olanzapine LAI (on intranet)
Paliperidone tablets Tablets NOT approved. (Long acting injection approved) 
Risperidone LAI Long acting injection - named patient request only (tablets approved). NO new initiations from Feb 2017. Patients commenced prior to this date can continue to receive this.
Trimipramine Removed from formulary as not a cost -effective choice of tricyclic antidepressant
Varenicline  Named patient request only. NICE -TA123 - Varenicline for smoking cessation
