HPFT are partnering with Patients Know Best (PKB) so that service users can have access to a free online portal which will help you access your appointment information, health records and get more involved in your recovery journey.

Initially you will be able to see your appointment information in PKB followed by extra functionality which will allow you to store and access your health records safely and securely. You can also use PKB to share information, such as your care plan with other healthcare teams, carers, family and friends and to track your progress over time.

This free service is accessible at anytime, whether this is on a mobile phone, tablet or computer, meaning your information is at your fingertips, whenever you need it 24/7.
