The Connected Lives model will put choice, independence, enablement and citizenship at the heart of everything we do. It gives us an opportunity to look at real and innovative solutions that enable people to live their lives to the full.

It is based on the core ideas behind the Care Act, ensuring people have choice, have access to opportunities in their local communities and can be active citizens of Hertfordshire. These will be key to delivering personalised social care in HPFT.
