Community learning disabilities

We are a team of Health & Social Care Professionals who support adults with Learning Disabilities.
The team offers specialist assessment, including diagnostic assessment of learning disabilities and autism.

Depending on what is needed the team work in different ways:
- Directly with the person
- Indirectly - with carers and families
- Jointly with other professionals    
- Signposting and support to access mainstream services

We care for adults (people aged 18 and over) with a learning disability whose needs cannot be met by mainstream services. Individuals must be registered with a Norfolk GP.

We are a multi-disciplinary team, this means we have lots of different kinds of professions:

- Learning Disability Nurses
- Clinical Psychologists
- Speech & Language Therapists
- Occupational Therapists
- Physiotherapists
- Dieticians
- Health Care Assistants
- Social Workers & Assistant Practitioners
- Intensive Support Team Psychiatry

 There are five Community Learning Disabilities Teams which cover Norfolk. We also see people at home, day services, in clinics, at GP surgeries and other settings as appropriate.
