NHS England published the findings of a report by Niche Patient Safety who were commissioned by NHS England to undertake an independent investigation into the homicide of a care worker that occurred in 2007 involving service user Mr P. The report can be viewed on NHS England's website  and our action plan based on the report's recommendations is attached.
