HPFT colleagues shortlisted for their outstanding work with care leavers

Two HPFT colleagues have been nominated for Partner of the Year Award in the 2026 Children’s Services Awards run by Hertfordshire County Council.

Jason and Kavita who are Care Leaver Mental Health Workers, were nominated for their outstanding contribution to the wellbeing, stability and long-term outcomes of the care leavers they support.

Their nomination highlights how they ‘advocate tirelessly for young people’s needs, working closely with clinical teams to avoid gaps in support and ensuring continuity of care. Their collaborative approach has significantly reduced waiting times, disengagement and crisis presentations during transitional periods.’


The nomination also shared ‘what truly sets these two workers apart is their compassion, patience, and unwavering belief in the potential of every young person. They build trust where it has often been broken, provide stability during moments of uncertainty, and offer hope when the future feels overwhelming. Their impact can be seen not only in improved mental health outcomes, but in the confidence, engagement, and optimism of the young people they serve.’

Jason and Kavita shared how ‘this recognition means a great deal to both of us.’

‘Working with care-experienced young people is a privilege.  We meet individuals who have faced challenges that most people never see — yet they show resilience, humour, courage and determination. They are the reason we do this work, and they are the ones who inspire us to keep pushing for better support, better understanding and better outcomes.’
