Digitising Patient Care Award

The Trust has been shortlisted for the Digitising Patient Care Award. Digitising assessment forms with Censeo (by Psyomics) prior to initial appointments means that clinicians are able to gather meaningful insights about individual service users’ experiences and background, enabling them to place more emphasis on the needs of the service user during the appointment.

Through this, we have been able to save up to 50% triage time – triage is a process of assessing service users and determining the order of priority for each individual’s treatment. We have also been able to save up to 25% clinical initial assessment time.

Towards Net Zero Award

HFPT has also been shortlisted for the Towards Net Zero Award. Through our team’s work on dementia inpatient wards, we have been able to introduce a formalised process to enable safe deprescribing decisions, rationalise meditations and promote the sustainable use of resources for older people approaching end of life.

The aim of this is to reduce the environmental impact of our healthcare system and reduce our carbon footprint without compromising on quality.

Workforce Initiative of the Year

Finally, we have been shortlisted for the Workforce Initiative of the Year for our Appraisal App.

Our Appraisal App is designed for staff and managers to guide and record appraisal conversations digitally and in real-time. Many valuable elements are built into this App, such as the opportunity to embed wellbeing conversations into the discussion.

The App improves productivity and will help the Trust to improve workforce development and retention.
