HPFT shortlisted at Nursing Times Awards for improving care for people with serious mental illness

The Trust is delighted to have been shortlisted at the Nursing Times Awards for an innovation that is improving safety and care for people with serious mental illness.
The Quality Improvement initiative introduced innovative electronic patient journey boards (EPJBs) across 10 adult community mental health services within nurse-led clinics that supports 1,320 service users and deliver around 22,000 clinic appointments each year , helping nurses monitor service users receiving depot and long-acting injectable antipsychotics (LAIs) and clozapine in real time.
Central to this was co-production of Fundamentals of Care Standards for nurse-led clinics by adult community matrons, community nurses, an Expert by Experience and other disciplines. Embedded within the EPJBs, they give nurses real-time visibility of medication, side-effect monitoring, living circumstances and wider care supporting earlier intervention and safer, more proactive care.
Replacing manual spreadsheets and retrospective audits with live data has enabled nurses to identify missed medication, overdue care and emerging risks much earlier, improving coordination, consistency and patient safety.
The innovation, which has been shortlisted in the Nursing in Mental Health category, has helped shift care from reactive to proactive, improving coordination, consistency and safety while supporting people to remain well at home.
